For the Plan of Action you will provide the 3 goals you have been working on with your client; then identify the strategies and the rationale that has been implemented. Adding the discovery gives your reader insight. Follow the guide below:
1. Make a plan in consultation with your citizen (Skills c).
2. Set one objective for each Dimension.
3. Establish strategies for reaching each objective.
For each objective, identify the criterion, the instrument, and the outside observer.
Throughout this process, continue to monitor your progress and use of knowledge (Self and Others d). Be sure to keep logs of your first meeting(s) with the citizen.
List your resources, and whatever you have been using along the way. List your short-term goals (14 weeks) the rationale of why you chose what you did and the strategies to achieve this goal and the outcome or presumed outcome.

This is an example of ONE GOAL. You will need three GOALS to create with your client.
Overall Goal: Sergio expresses one Goal as wanting to learn how to control his anger “ I want to learn to control my anger.”
Counseling Goal 1: Sergio will be able to demonstrate a capacity to decrease his verbal outburst with people.
Objective 1: Sergio will learn what triggers his anger
Assist Sergio to identify what makes him really mad
Ask Sergio to talk about three times he can remember being really angry which has got him in trouble.
Sergio was able to talk about the situations but needed support in identifying what triggered his anger.
Encourage Sergio to identify three times when someone did not control his anger with him.
Sergio could easily ID these situations and needed prompting to talk about how it made him feel.
Develop cards that help Sergio identify his triggers (person, situation, place, feelings).
With moderate assistance, Sergio wrote down three triggers.
Create a log with Sergio so he could write down when he thinks he “lost control of his anger.”
Discovery: Sergio liked the idea of the log and with a daily reminder was able to write in it.
Objective 2: Sergio will learn skills and techniques to calm his anger
Strategy/Intervention: Methods on how to calm angry feelings will be implemented
Rationale: Intra-conflict resolution training
· Assist Sergio in writing down three things that may calm him.
· With minimum assistance, Sergio could think of three things that calmed him.
· Practice the three things with Sergio.
· Roleplay with Sergio a situation that would trigger his anger and work it out.
· Sergio, with lots of coaching, was able to practice his calming techniques.
Discovery (Sergio had some trouble with the role-playing as he felt that he could not get angry spontaneously)
Objective 3: Assist Sergio to create a log to see patterns of progress and triggers
· Review Sergio’s log to help him understand if there is a pattern and look at progress.
· Create a graph to see when he is more irritable and stressed angered
· Develop more of an understanding and awareness of his triggers
Discovery: (With moderate cueing Sergio was able to recognize that he tends to feel most angry in the morning.
( My plan of action should consist of me working with a mentally ill client who struggles with being stable and obtaining permanent housing while temporarily being housed in a shelter which contain 200 men who struggles with mental illness and sobriety. This particular client I am working with is 31 years old African American male and one of his goals should be him maintain and adhering to his current medication so he can be able to sustain permanent housing on his own. another goa is working on his behaviors.
