Case Study
In 2016, Ernesto Bautista, aged 89 years, and his wife Camille, aged 87, sold their small restaurant and immigrated to Miami, Florida, from a small town in the Visayan region of the Philippines. They came to join their firstborn daughter, a nurse named Leticia, aged 53; her husband; and their three children. Mr. Bautista speaks limited English and is in a post-stroke rehabilitation unit. Since the stroke, he has had mild aphasia, mild confusion, and bladder and bowel continence problems. His hypertension and long-standing diabetes are controlled with medication and diet. His wife, daughter, and grandchildren have been supportive of him during this first hospitalization experience. Mr. Bautista’s family has cooperated with the health team, often agreeing with minimal resistance to the prescribed treatment. The rehabilitation team recommended subacute rehabilitation treatment as part of the discharge plan.
As a business man and the elder in the family household, Mr. Bautista is looked to for counsel by the immediate and extended family. Mr. Bautista’s status, however, has caused friction between Leticia and her husband, Michael, an American-born Filipino, who works as a machinist. Michael has accused Leticia of giving excessive attention to her mother and father. Leticia’s worries about her parents’ health have made Michael very resentful. He has increased his already daily “outings with the boys.” Leticia maintains a full-time position in acute care and part-time night-shift position in a nursing home.
Mr. Bautista’s discharge is pending, and a decision must be made before Medicare coverage runs out. Leticia has to consider the possible choices available to her father and the family’s circumstances and expectations. Mrs. Bautista, who speaks very limited English and is being treated for hypertension, has always deferred decisions to her husband and is looking to Leticia to make the decisions. Because of her work schedule, the absence of a responsible person at home, her mother’s health problems, and intergenerational friction, Leticia considers nursing home placement. She is, however, reluctant to broach the subject with her father, who expects to be cared for at home. Mrs. Bautista disagrees with putting her husband in a nursing home and is adamant that she will care for her husband at home.
Leticia delayed talking to her father until the rehabilitation team requested a meeting. At the meeting, Leticia indicated that she could not bring herself to present her plan to put her father in a nursing home because of her mother’s objection and her own fear that her father will feel rejected. Feeling very much alone in resolving the issue about nursing home placement, she requested the team to act as intermediary for her and her family.
Case Study Questions
Identify cultural family values that contribute to the conflicts experienced by each family member.
Identify a culturally competent approach the team can use when discussion nursing home placement with the Bautistas.
How might the rehabilitation program be presented to Mrs. Bautista and still allow her to maintain her spousal role?
Discuss at least three communication issues in the family that are culture-bound, and suggest possible interventions.
Identify and explain major sources of stress for each member of the household and suggest strategies to manage their stress.
